Our nurses will be counted on to develop individualized treatment plans to address the specific needs each A&T Healthcare patient has. Responsibilities associated with our nursing jobs include wound care, physical therapy, IV therapy, medication management, nutritional assistance, and social work, among others.
RN – Field Clinical Supervisor
Oversee care delivery in the home by professionals and paraprofessionals. Provide direct skilled care such as wound care. Home care and/ or hospital experience preferred.
RN/LPN
Private duty in the home. Adult and pediatric. Experience with G-tube, tracheostomy and ventilator preferred.
Home Health Aide
HHA certificate required, or take our FREE HHA class. Provide care to individuals in their home.
Personal Care Aide
PCA certificate or experience in providing personal care to individuals in their home. We also offer FREE PCA classes.
Physical Therapist
Physical therapists are responsible for evaluating and assessing the needs of patients and formulating a plan for recovery in a home environment.

Service Coordinator
Coordinate all aspects of care for individuals in the Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) programs. Knowledgeable regarding Medicare/Medicaid, case management experience preferred.
FREE Home Health Aide or Personal Care Aide classes
